More than 4. 0 years on, Weight Watchers International claims to have helped millions of people around the world to lose weight. Last year alone, 6,5. UK each week – around 2. But ironically, as the Weight Watchers emporium continues to grow, so too, do our waistlines. The question is, can Weight Watchers really help us lose weight and more importantly, keep it off? How does Weight Watchers work? Weight Watchers effectively promotes a reduced- calorie, low- fat diet, to encourage weight loss. The Weight Watchers programme is called Switch and consists of two different diets: The Points Plan – this allows you to eat and drink anything you like as long as you stick to a daily Points allowance. There’s no limit on quantity – you simply eat until you feel comfortable. You also have a weekly Points allowance for treats. You can . Points are calculated according to the calorie and saturated fat content of foods and the portion size. However, the exact formula used is kept a closely guarded secret by Weight Watchers. As a guideline though, the more calories a product contains, the higher its Points value. With the Point system, you simply add up the Points values of the foods you eat and drink in a day and make sure you don’t go over your daily Points allowance. So how do I know how many Points I’m allowed? Your group leader will advise you on this at your first meeting. Alternatively, if you become an online member of Weight Watchers, you’ll be advised on the number of Points you should have each day after entering your personal details. Your daily Points allowance is based on your weight, height, gender, age and level of activity during the day. In general, the more weight you have to lose, the more Points you are allowed each day. For example, a 3. Points a day. If that same person weighed 1. Points a day. On average, most people have 2. Points a day. What if I don’t know how many Points there are in a food? Members who attend weekly meetings receive free Points finders to help them calculate the Points from food using calories and saturated fat details. ![]() ![]() Also materials handed out at meetings when you first join contain basic food lists. Members can also buy a Points calculator and/or a Shopping Guide with the Points values for branded and supermarket foods. Weight Watchers on line members and members who attend weekly meetings but are also signed up for internet support (e. Source) have online access to a database that includes the Point values for almost 3. Points calculator. What if I’m really active? Doing more exercise earns you more Points. For example, 2. 0 minutes of aerobics will add an extra 1 Point to your weekly total, whereas 2. Points. You can earn as many activity Points in a week as you wish. However, you are only allowed to spend 1. Points each week on food. The idea is to earn more than you eat. Tell me more about Weight Watchers Core Plan? Quite simply, you eat as much as you want from a list of . You are then allowed a weekly allowance of 2. Points, which you can use for treats or snacks. Weight Watchers Core Plan is based on foods that a have a low calorie value relative to their actual weight, in other words, they have a low energy density. According to Weight Watchers, research shows that people tend to eat the same weight of food each day, regardless of the calories it contains. So by eating mostly foods with a low energy density you'll feel satisfied on fewer calories. What weight loss results did you achieve on your DAMY Program? At the same time you’re encouraged to use a scale to help you identify feelings of hunger and fullness. How does Weight Watchers online membership compare to Weight Loss Resources? There are many differences. For example, Weight Watchers doesn’t allow you to opt to lose weight more slowly, for example, 1lb or 1. Points allowance. ![]() Plus, there’s no indication given about how long it will take you to reach your goal. Unlike WLR, when you complete the Weight Watchers food diary there’s no information provided about the daily number of servings of fruit and veg you’ve had, the amount of water you’ve consumed or the amount of calories and grams of fat and fibre you’ve had – all things that help to highlight whether or not you’re eating a healthy, balanced diet. The main difference however, is the lack of information about calories, even for recipes. The entire focus is on Points. This means you need to calculate the Points values of pretty much everything you eat and drink – and of course, if you have to log on to do this or search for your Points calculator, it’s more time consuming than simply reading the calorie information on the back of food packets. What are the pros? No foods are banned! If you follow the Points plan, you can eat or drink whatever you like, providing you stick to your daily Points allowance. There are more restrictions for the Core Plan, but ultimately, you’re still allowed treats. Weight Watchers could be a good choice if you don’t like the hassle of counting calories – although you need to be aware that you’ll still have the hassle of working out the Points values of many foods! Ultimately, both the Points Plan and Core Plan encourage you to eat more healthily – the Core Plan only lets you chose from healthier foods with a few treats each week, while on the Points Plan, if you want to stick to your daily allowance you need to opt for lower- calorie, lower- fat foods such as fruit, veg, chicken, lean meat and wholegrain cereals. Weight Watchers weekly meetings can be a good choice if you find it hard to diet on your own and like the idea of having support from others on a personal level. ![]() They also provide opportunities for making new friends. And the cons? Whether you opt for weekly meetings or Weight Watchers online, it’s pricey, and if either of the diets don’t suit you, you’ll end up losing pounds from your purse rather than your waistline. It can also be just as time consuming – if not more so – to work out the Points values of foods than it would be to simply count calories. Many of the Points values included in the Weight Watchers database are based on portions rather than actual weights. Whilst there’s a portion size chart to help you identify what counts as, for example, a . Meanwhile, with the Core Plan it’s still possible to consume large amounts of . Alternatively, you can become an online member. Members who attend weekly meetings also have the option of receiving online support at any time of night or day with e. Source, although you pay extra for this service. Finally, there is a Weight Watchers At Home service that provides you with monthly information by post. Here’s some more information on each. ![]() ![]() At your first meeting, you’ll agree your weight loss goals with your leader and find out more about the two eating plans. If you decide to try the Points Plan your group leader will calculate your daily Points allowance. Each week you’ll have a confidential weigh- in and your group leader will lead a 3. You’ll also have the support of other members to help you stay motivated. Which weight loss plan will give you the best results? Read the reviews of the diets you might want to follow (Photo: Getty). An online membership costs an initial subscription fee of . This effectively provides weekly members with all the same information they’d receive if they were online members. In addition to the fee for the weekly meetings, e. Source costs . All the information is sent to you via post. Leaders go through an interview process with the area manager and if successful, attend weekend workshops for intensive training. They then shadow existing leaders before running their own meetings. All leaders have eight training sessions a year with their regional trainers and area manager, plus annual training with the national training team. Group leaders have all lost weight with Weight Watchers, too, so have had first hand experience of the diets and the highs and lows linked with trying to lose weight. Most cases are not severe enough to affect general health and are treated in the outpatient setting. Rare life- threatening presentations can occur that require intensive inpatient management. This topic reviews the treatment of psoriatic skin disease. The epidemiology, clinical manifestations, and diagnosis of psoriatic skin disease are discussed in detail separately, as are psoriatic arthritis and the management of psoriasis in pregnant women and special populations. Therefore, management of psoriasis involves addressing both psychosocial and physical aspects of the disease. ![]() ![]() Crohn's disease; Synonyms: Crohn syndrome, regional enteritis: The three most common sites of intestinal involvement in Crohn's disease are. Psoriasis is a common chronic skin disorder typically characterized by erythematous papules and plaques with a silver scale, although other presentations occur. Numerous topical and systemic therapies are available for the treatment of the cutaneous manifestations of psoriasis. Treatment modalities are chosen on the basis of disease severity, relevant comorbidities, patient preference (including cost and convenience), efficacy, and evaluation of individual patient response . Although medication safety plays an important role in treatment selection, this must be balanced by the risk of undertreatment of psoriasis, leading to inadequate clinical improvement and patient dissatisfaction . The clinician needs to be empathetic and spend adequate time with the patient. It may be helpful for the clinician to touch the patient when appropriate to communicate physically that the skin disorder is neither repulsive nor contagious. ![]() Clinicians should lay out reasonable aims of treatment, making it clear to the patient that the primary goal of treatment is control of the disease. Although treatment can provide patients with high degrees of disease improvement, there is no cure for psoriasis. Educating the patient about psoriasis is important and referral to an organization such as the National Psoriasis Foundation (www. Psoriasis may affect patients' perceptions of themselves and this can potentially initiate or exacerbate psychological disorders such as depression . Patients with limited skin disease may still have significant psychosocial disability . Some patients with psoriasis may benefit from counseling and/or treatment with psychoactive medications. Choice of therapy — For most patients, the initial decision point around therapy will be between topical and systemic therapy. However, even patients on systemic therapy will likely continue to need some topical agents. Topical therapy may provide symptomatic relief, minimize required doses of systemic medications, and may even be psychologically cathartic for some patients. For purposes of treatment planning, patients may be grouped into mild- to- moderate and moderate- to- severe disease categories. Limited, or mild- to- moderate, skin disease can often be managed with topical agents, while patients with moderate- to- severe disease may need phototherapy or systemic therapy. The location of the disease and the presence of psoriatic arthritis also affect the choice of therapy. ![]() ![]() ![]() Psoriasis of the hand, foot, or face can be debilitating functionally or socially and may deserve a more aggressive treatment approach. The treatment of psoriatic arthritis is discussed separately. Patients with more than 5 to 1. Attempts to treat extensive disease with topical agents are often met with failure, can add cost, and lead to frustration in the patient- clinician relationship. There is ample evidence of efficacy of the newer systemic therapies (. Established therapies such as methotrexate and phototherapy continue to play a role in the management of moderate to severe plaque psoriasis. However, the availability of biologic medications has reduced the challenge considerably. The concept that many patients with psoriasis in the United States do not receive sufficient treatment to control the disease is suggested by an analysis of surveys performed by the National Psoriasis Foundation between 2. Among the 5. 60. 4 survey respondents with psoriasis, 5. Many patients received no treatment, including 3. Further studies will be useful for clarifying the reasons for these observations and for determining the value of interventions to increase the accessibility of treatment. Widespread pustular disease requires aggressive treatment, which may include hospitalization. ![]() Therapeutic approaches to generalized pustular psoriasis and psoriatic arthritis are discussed separately. Alternatives include vitamin D analogs, such as calcipotriene and calcitriol, tar, and topical retinoids (tazarotene). For facial or intertriginous areas, topical tacrolimus or pimecrolimus may be used as alternatives or as corticosteroid sparing agents, though improvement may not be as rapid. Localized phototherapy is another option for recalcitrant disease. Combinations of potent topical corticosteroids (table 1) and either calcipotriene, calcitriol, tazarotene, or UVB phototherapy are commonly prescribed by dermatologists. Calcipotriene in combination with Class I topical corticosteroids is highly effective for short- term control. Calcipotriene alone can then be used continuously and the combination with potent corticosteroids used intermittently (on weekends) for maintenance. A combination product containing calcipotriene and betamethasone dipropionate is available for this use. With proper adherence, considerable improvement with topical therapies may be seen in as little as one week, though several weeks may be required to demonstrate full benefits. Because adherence to topical treatment can be a major hurdle, keeping the treatment regimen simple and using treatment vehicles that the patient finds acceptable is often beneficial . Biologic agents used in the treatment of psoriasis include the anti- TNF agents adalimumab, etanercept, and infliximab, the anti- interleukin (IL)- 1. IL- 1. 7 antibody secukinumab. Improvement usually occurs within weeks. Patients with severe psoriasis generally require care by a dermatologist. Intertriginous psoriasis — Intertriginous (inverse) psoriasis should be treated with class VI and VII low potency corticosteroids (table 1) due to an increased risk of corticosteroid- induced cutaneous atrophy in the intertriginous areas. Topical calcipotriene or calcitriol and the topical calcineurin inhibitors tacrolimus or pimecrolimus are additional first- line treatments . These agents may be used alone or in combination with topical corticosteroids as corticosteroid sparing agents for long term maintenance therapy. Calcipotriene, tacrolimus, and pimecrolimus are more expensive options than topical corticosteroids. Some concerns have been raised about the safety of the calcineurin inhibitors (see 'Calcineurin inhibitors' below and . Scalp psoriasis — The presence of hair on the scalp can make topical treatment of psoriasis challenging because patients may find certain products messy or difficult to apply. Recognizing the patient's preference for a drug vehicle may help to improve adherence to therapy. For many patients, lotion, solution, gel, foam, or spray vehicles are preferable to thicker creams or ointments. Topical corticosteroids are the primary topical agents used for psoriasis on the scalp . Support for the use of these agents is evident in a systematic review of randomized trials that found that very potent or potent topical corticosteroids are more effective treatments for scalp psoriasis than topical vitamin D analogs . Combining a corticosteroid and vitamin D analog may offer additional benefit; in the systematic review, combination treatment with a potent topical corticosteroid and a vitamin D analog appeared slightly more effective than potent topical corticosteroid monotherapy. However, in clinical practice, complicating the treatment regimen with more than one topical product may reduce the likelihood of consistent adherence to the treatment regimen. Thus, we usually prescribe a topical corticosteroid alone as initial therapy. Commercial betamethasone dipropionate- calcipotriene combination products are available, but are more expensive than most topical corticosteroid preparations. Other topical therapies used for psoriasis (eg, tazarotene, coal tar shampoo, anthralin) and intralesional corticosteroid injections also may be beneficial for scalp involvement, though data on efficacy specifically in scalp disease are limited . Salicylic acid can be a helpful adjunctive treatment because of its keratolytic effect. Phototherapy (eg, excimer laser) and systemic agents are additional treatment options for patients who cannot achieve sufficient improvement with topical agents . Approaches include potent topical corticosteroids and topical bath psoralen plus UVA phototherapy (PUVA). However, these drugs appear to be particularly effective in the treatment of pustular psoriasis, and we consider them first line therapy. Acitretin is the retinoid that is used most often for this indication. Acitretin is a potent teratogen and should not be used in women who might become pregnant. Pregnancy is contraindicated for three years following acitretin therapy. The management of nail psoriasis is reviewed in detail separately. Based upon data from open- label or retrospective studies and case reports, a panel of experts suggested that patients with severe, unstable disease should be treated with cyclosporine or infliximab due to the rapid onset and high efficacy of these agents . Patients with less acute disease can be treated with acitretin or methotrexate as first- line agents. The panel advised against the use of systemic glucocorticoids due to the perceived potential for these drugs to induce a flare of psoriasis upon withdrawal of therapy. Etanercept was effective in an open- label study of 1. Topical therapies, such as mid- potency topical corticosteroids, emollients, wet dressings, and oatmeal baths can be used in concordance with systemic treatment to manage symptoms . Long- term maintenance therapy for psoriasis is required. Children — The immediate and long- term adverse effects of therapies for psoriasis are of particular concern in the pediatric population. Newsweek reported that seven- month- old Lucas’s parents, identified only as Peter S. The owners of a health food store, the baby’s parents wanted to feed their son a vegan diet and eschewed the usual cow- based baby formula in favor of their own vegetable milk concoction. However, it is commonly held that babies need certain specific nutrients in order to grow and develop, and these nutrients are not contained in vegetable milk. While a vegan diet may be suitable for consenting adults, it is not at all sufficient to sustain a human baby, as shown by the tragic death of baby Lucas. However, it’s not just the parents’ unfortunate choice of diet for their young son that was to blame for the boy’s death. The court, which went on to convict the parents of the death of their son, held that the boy’s death was caused by the “systematic offer of food which was not suitable.” The court also found that the parents ignored all warning signs that their young son was in medical trouble. CBS News reported that at the time of his death, baby Lucas weighed just nine pounds, much lower than the acceptable weight for a 7- month- old boy. ABC News added that the baby had gained only 2. In addition to his small size and weight, the parents allegedly opted not to seek medical attention when their son was “gasping for air” in the days leading up to his eventual death. After he died, it was found that the baby’s stomach was completely empty and his organs had shrunk to half their usual size. ![]() Not only did the parents fail to seek medical care for their son when he showed signs of dehydration, malnutrition, and breathing problems in the days leading up to his death, Peter S. While finding the couple guilty of the death of their son, the court nevertheless conceded that they had “truly loved” their child and that it was for this reason that a lenient six- month suspended sentence was imposed.“It is clear that the defendants have already been severely punished because they need to proceed with life in the knowledge that they are responsible for the death of their son, who they truly loved.”.

Contrary to what you (or your trainer) might believe, endorphins aren’t responsibly for that giddy exuberance you feel after a long run. What is associated with the sought- after feeling is something that gets you actually high: cannabis. Specifically, we’re talking about a chemical in the endocannabinoid family. Somewhat similar to your traditional cannabinoids (like THC and CBD) found in marijuana, endocannabinoids are made within the body. Michael Aranda, host of the Sci. Show You. A over the Soviet Union by a surface- to- air missile (SAM). Another U- 2, piloted by Major Rudolf Anderson, Jr., was lost in a similar fashion during the Cuban Missile Crisis of 1. The U- 2 is one of a handful of aircraft types to have served the USAF for over 5. The newest models (TR- 1, U- 2. R, U- 2. S) entered service in the 1. The current model, the U- 2. Into the 1. American government had on facilities deep inside the Soviet Union were German Luftwaffe photographs taken during the war of territory west of the Ural Mountains, so overflights to take aerial photographs of the Soviet Union began. After 1. 95. 0, Soviet air defenses aggressively attacked all aircraft near the country's borders—sometimes even those over Japanese airspace—and the existing reconnaissance aircraft, primarily bombers converted for reconnaissance duty such as the Boeing RB- 4. Richard Leghorn of the USAF suggested that an aircraft that could fly at 6. Mi. G- 1. 7, the Soviet Union's best interceptor, which could barely reach 4. He and others believed that Soviet radar, which used American equipment provided during the war, could not track aircraft above 6. The British had already produced the PR3 photo- reconnaissance variant, but the USAF asked for English Electric's help to further modify the Martin B- 5. American licensed version of the Canberra) with long, narrow wings, new engines, and a lighter- than- normal airframe to reach 6. Air Research and Development Command mandated design changes that made the aircraft more durable for combat, but the resulting RB- 5. D aircraft of 1. 95. The Soviet Union, unlike the United States and Britain, had improved radar technology after the war, and could track aircraft above 6. The USAF decided to solicit designs only from smaller aircraft companies that could give the project more attention. Officials at Lockheed Aircraft Corporation heard about the project and decided to submit an unsolicited proposal. To save weight and increase altitude, Lockheed executive John Carter suggested that the design eliminate landing gear and avoid attempting to meet combat load factors for the airframe. The company asked Clarence . Johnson was Lockheed's best aeronautical engineer. He was also known for completing projects ahead of schedule, working in a separate division of the company, informally called the Skunk Works. The design was powered by the General Electric J7. It could reach an altitude of 7. ![]() In June 1. 95. 4, the USAF rejected the design in favor of the Bell X- 1. B- 5. 7. Reasons included the lack of landing gear, use of the J7. Pratt & Whitney J5. Bissell, Jr., that his agency should fund and operate this aircraft. Land believed that the military operating the CL- 2. Although Dulles remained reluctant to have the CIA conduct its own overflights, Land and James Killian of MIT told President Eisenhower about the aircraft; Eisenhower agreed that the CIA should be the operator. Dulles finally agreed, but some USAF officers opposed the project because they feared it would endanger the RB- 5. D and X- 1. 6. The USAF's Seaberg helped persuade his own agency to support the CL- 2. J5. 7 engine, and final approval for a joint USAF- CIA project (the first time the CIA dealt with sophisticated technology) came in November 1. ![]() Lockheed had meanwhile become busy with other projects and had to be persuaded to accept the CL- 2. Lockheed received a $2. March 1. 95. 5 for the first 2. Johnson's home in February 1. The company agreed to deliver the first aircraft by July of that year and the last by November 1. It did so, and for $3. When Johnson ordered altimeters calibrated to 8. CIA set up a cover story involving experimental rocket aircraft. Shell Oil developed a new low- volatility, low vapor pressure jet fuel that would not evaporate at high altitudes; the fuel became known as JP- 7, and manufacturing several hundred thousand gallons for the aircraft in 1. Esso's FLIT insect repellent. The aircraft was renamed the U- 2 in July 1. Article 3. 41, was delivered to Groom Lake. The new camera had a resolution of 2. This effort ultimately proved unsuccessful, and work began on a follow- on aircraft, which resulted in the Lockheed A- 1. Oxcart. Disclosed details say the design is essentially an improved U- 2 airframe with the same engine, service ceiling, sensors, and cockpit, with the main differences being an optional manning capability (something Lockheed has proposed for the U- 2 to the Air Force several times but has never gained traction) and low- observable characteristics. The Air Force has no requirement or timeframe for a next- generation High- Altitude Long Endurance (HALE) platform, but Lockheed sees a future need and wants something in development early. Having the option of an onboard pilot is considered a deterrent because it can be used in peacetime situations where unmanned aircraft would more likely be engaged, since there is no possibility of killing a person. The company's last attempt to create a stealth unmanned aircraft was the RQ- 3 Dark. Star, which never made it past flight testing and was canceled. Its name was changed to mean . Size, and thus cost, is kept down by having less endurance than the Global Hawk at around 2. RQ- 4 sortie even though it is capable of flying for 3. Although originally planned to be optionally- unmanned, some Lockheed officials are leaning towards a purely unmanned aircraft, as it is expected that issues with airspace integration of UAVs will be addressed by the time it will be introduced. The TR- X concept is aimed squarely at Air Force needs, and is not currently being marketed to the CIA or other government agencies. It would have increased power and cooling to accommodate new sensors, communication equipment, electronic warfare suites, and perhaps even offensive or defensive laser weapons. TR- X could be ready for service in the 2. U- 2s and RQ- 4s. Its maximum takeoff weight would be greater than either the U- 2's or RQ- 4's at around 5. It will use the same F1. U- 2, but thrust could increase to 1. VA; service ceiling would increase to 7. The TR- X is meant to be . JPTS is a high thermal stability, high altitude fuel, created specifically as fuel for the U- 2. JPTS has a lower freeze point, higher viscosity, and higher thermal stability than standard Air Force fuels. In 1. 99. 9, the United States Air Force spent approximately $1. U- 2 aircraft and was looking for a lower cost alternative. JPTS is a specialty fuel and as such has limited worldwide availability and costs over three times the per- gallon price of the Air Force's primary jet fuel, JP- 8. Research is under way to find a cheaper and easier alternative involving additives to generally used jet fuels. A JP- 8 based alternative, JP- 8+1. LT, is being considered. JP- 8+1. 00 has increased thermal stability by 1. Martin Knutson said that it . The U- 2. S and TU- 2. S variants incorporated the even more powerful General Electric F1. To maintain their operational ceiling of 7. U- 2. A and U- 2. C models had to fly very near their never- exceed speed (VNE). The margin between that maximum speed and the stall speed at that altitude was only 1. This narrow window is called the . A stall would cause a loss of altitude, possibly leading to detection and overstress of the airframe. However, at lower altitudes, the higher air density and lack of a power assisted control system makes the aircraft very difficult to fly. Control inputs must be extreme to achieve the desired response, and a great deal of physical strength is needed to operate the controls in this manner. The U- 2 is very sensitive to crosswinds, which, together with its tendency to float over the runway, makes the aircraft notoriously difficult to land. As it approaches the runway, the cushion of air provided by the high- lift wings in ground effect is so pronounced that the U- 2 will not land unless the wing is fully stalled. A landing U- 2 is accompanied on the ground by a chase car and an assisting U- 2 pilot calling off the angles and decreasing aircraft height as the aircraft descends. The rear wheels are coupled to the rudder to provide steering during taxiing. To maintain balance while taxiing, two auxiliary wheels, called . These fit into sockets underneath each wing at about mid- span, and fall off during takeoff. To protect the wings during landing, each wingtip has a titanium skid. After the U- 2 comes to a halt, the ground crew re- installs the pogos one wing at a time, then the aircraft taxis to parking.
